Care Navigation

The Care Navigation team is dedicated to helping you navigate and answer any questions you may have regarding finding the right provider, specialist, or service to meet your health care needs.

• Schedule appointments, labs, and preventative screenings.
• Locate Premier providers, facilities, and services.

Care Management

The Care Management team provides personalized, comprehensive care management and coordination services to employees and their families who are enrolled in the RWJBarnabas Health medical plans, with HIPAA-compliant confidentiality.

Our multidisciplinary team of nurses, social workers, and care coordination support specialists are available for:

• Transitions of Care Services for coordination of care and follow-up before, during, and after hospitalization.
• Diabetic Care Management services include assistance in management of provider referrals, individualized treatment goals, review of medications, access to community/social services, and patient education.
• Connect you with hospital-based resources, community-based resources, or the appropriate resources to assist you with your benefit and claim issues.
