Benefits
The 1095-C forms for Year 2025 have been distributed. If you have opted in to electronic forms from WEX, our Benefits administrator, you may access your 1095-C form in the Benefits Enrollment Portal.
We offer the choice of two plan options administered by Delta Dental™ – the Base Plan and the Buy-Up Plan. You can find the plan designs here:
Delta Dental™ has two in-network tiers covered under the plans, collectively called the “Delta PPO™ Plus Premier Network.” Delta Dental’s network is the largest in the country with 155,000+ participating dentists at 400,000+ locations. The PPO Network has the lowest out-of-pocket cost. The Premier Network has slightly higher out-of-pocket costs but still includes generous discounts.
While both the Base Plan and Buy-Up Plan also offer out-of-network coverage, using an in-network dentist will be most cost effective for you. In-network dentists have agreed to pre-set fees, so you won’t get surprise bills for the difference between what they charge and what your plan covers.

To find a participating dentist, log onto Delta Dental™.
Finding Your ID Card:
You should have received a hard copy of the Dental ID card in the mail in December. To access the digital version of the ID card, log in to the Delta Dental™ portal here or download the Delta Dental™ mobile app. Note: When registering, please remove spaces from your name (e.g., type ‘MaryAnn’ instead of ‘Mary Ann’). Below is a sample of the ID card.
