Horizon Dental Plan Options
Note Regarding the DHMO Plan:
The DHMO Plan requires that you select a Primary Care Dentist who coordinates all your care and arranges for in-network referrals to specialists. This plan only provides in-network benefits.
Finding Participating Dentists
To find a participating dentist, log onto Horizon. For the Base or Buy-Up plans, search for Dentists in the Horizon Dental Option plan. For the DHMO plan, select Horizon Dental Choice plan.
